Make sure you also bring an ID and insurance card/s to your appointment. If you submit the paperwork online, please read the Notice of Privacy Practices (Online) (PDF).

So that your previous dentist can share your records and X-Rays, please send the Authorization to Release Information Form to your old dentist or send it to us and we will contact your the dentist.
